Welcome to the CARE Coalition!

In 2023, Touchstone Health Services was awarded a five-year grant through the Office of Population Affairs, Department of Health and Human Services. The grant looks to advance adolescent health through the replication of evidence-based programs (EBPs) and services in seventeen zip codes that incorporate the cities of Avondale, Tolleson, Goodyear, Litchfield Park, and parts of Glendale, Buckeye, and Phoenix.

Touchstone will partner with community agencies and organizations to provide a comprehensive approach to addressing the needs of youth within the community. The goal will be to collaborate with youth, caregivers, schools, community organizations, foster care systems, youth-serving agencies, after-school care providers, local city councils, faith-based organizations, and others to provide a comprehensive approach to support youth in building protective factors that mitigate risk behaviors.
