Welcome to the CARE Coalition!

In 2023, Touchstone Health Services was awarded a $1.5 million five-year grant through the Office of Population Affairs, Department of Health and Human Services. The grant looks to advance equity in adolescent health through the replication of evidence-based programs (EBPs) and services in seventeen zip codes that incorporate the cities of Avondale, Tolleson, Goodyear, Litchfield Park, and parts of Glendale, Buckeye, and Phoenix.  Touchstone will partner with community agencies and organizations to provide a comprehensive approach to addressing youth’s needs within the community. The goal will be to collaboratively work with youth, caregivers, schools, community organizations, foster care systems, youth-serving agencies, after-school care, local city councils, faith-based, and others to provide a comprehensive approach to support youth in building protective factors to mitigate engagement in risk behaviors.
